1. A trucking service that deals with transporting of agricultural products from the warehouse to the port has a delay tolerance of 4 hours. The company made sure to plan out all available routes that can be taken. 2 routes have span of 15km while the other 2 have 21-22Km. The shorter distance passes thru low areas as surveyed and as manifested by 4 bridges along their ways. The 21km road has 2 bridges, with one area surely to gets flooded during torrential rains. The last road which is 22km has 1 short bridge, flooding is still imminent in 2 areas, but only low floods which could reach up only to 1m.

Some possible answers:
- Lower bridge capacity – Distribute products in lower capacity trucks for it to be able to pass and also contingency if something happens to the other trucks.
- Not enough man power to support requirement – This might be one of the difficulties that we cannot avoid, so ensure to have a regular line of communications with the client. Also to mitigate impact, have a more conservative lead time, ensure delivery 1-2 days before deadline.

Actual Incidents
- During a typhoon, one of the authority in one of the university in one city of the Philippines, out of concern for the safety of the students and some of the personnel, requested them to stay in the dormitory instead of them going home. As the typhoon hit the city, the river beside the dormitory swell, and swallow the dormitory. Everyone that occupied the dorm drowned.
- During the strongest typhoon in history of the world, not heeding the warning that the typhoon would be catastrophic and just considering it as the usual typhoon that usually passes the area, the coastal people stayed in their homes instead of evacuating.
- A fire hit a disco in Metro Manila during a graduation celebration, panic broke out, there was not enough fire extinguishers, the door’s open direction is inward, there was no adequate and visible instructions/ directions to the fire exits, hundreds died.
- A data center was flooded when structural integrity failed. Not only some servers were destroyed, a life was also lost due to water getting in contact with power lines.
- A main water pipe in a BPO leaked, not only did it caused evacuation of one area of the floor, but also it came in contact with power lines so the company had to shut down the main floor power switch.
- During an emergency evacuation brought by a prank bomb threat, the canteen kitchen released a lot of smoke brought by a broken vent which caused panic to evacuees. This caused injuries due to stampede.
- Due to the failure of a production system, backup data were recovered from tapes, but the recovered data were unusable.
